Yes, if you or your family member asked for or if you already get mental health, developmental disability, or substance abuse (MH/DD/SA) services through Medicaid.

Yes, if you have asked for or if you already get services through one of the CAP Waivers.

Yes, if you have asked for medical equipment from Medicaid or from one of the CAP waivers.

No, if you have asked for a state-funded service. Appeals of state-funded services are handled by your LME.
